XIII. Appendixes

The need for complete documentation generally dictates the inclusion of appropriate appendixes in proposals.

The following materials are required for the appendix:

Using the ASU templates:

Recruitment letters sent to appropriate stakeholders (administrators or those who can permit participation of teachers, students, etc.).

Official letters of permission to conduct research.

Letters of consent per instrument using the ASU templates

Instruments:

Submit all your instruments, including the consent letters, as appropriate by type of participants.

Include a table per instrument that describes whether these are original

instruments developed by you or whether an instrument is copyrighted. If so, carefully cite the reference to the work, the instruments must be publicly available, or in some instances, permission in writing to reproduce the instrument from the copyright holder or proof of purchase of the instrument may be needed.
