- Every facility that is funded using Medicare or Medicaid funds is reviewed by the Federal Government to allow patients and families to advocate for and make good choices for their care.

- Select the type of facility from the left menu.
- Enter the address and name of the above facility.
- Click on the facility and review the government information for the facility listed above.
- Click on View Quality Measure and then click on the down arrow to view the Short Term and Long-Term measures.
