The patient is a 36-year-old female presenting to the clinic referred by PCP, she describes that ”her brain doesn’t feel right” since having her baby. The patient gave birth to her first child 4 weeks ago. The patient says she is lacking energy, not eating much, is very anxious when she hears the baby crying and she has trouble sleeping, she sleeps 3 hours a night. The patient states that she cries all the time and she is sad. The patient says that her mom lives with her and helps her with taking care of the baby but still feels overwhelmed. The patient says that she does not want to hold her baby or even see the baby sometimes. She feels that she is not a good mother because every time she holds the baby he starts crying. The patient does not live with a partner, and the father does not want to be involved and that is one reason for her anxiety. The patient states that she is not interested in watching movies or going out for a walk anymore. Denies suicidal and homicidal ideation. EPDS score is 20.

-past medical history of hypothyroidism taking levothyroxine 75 mcg
-sister and mother have a history of depression
-Father has a history of bipolar
