Problem: Depression and suicidal ideation
Patient 22-year-old african american male who has had 5 hospital stays for suicide attempts (mental health hospital) over the course of 7 years. He currently takes Zoloft 50 mg daily and receives Invega monthly injections. The patient attends weekly therapy sessions with a social worker, has a case manager, and sees a psychiatrist once a month.
Diagnosis: Schizoaffective disorder bipolar type.
Complaints: reluctance to take pills due to prior suicide attempts with pills. Invega does not last a full month. A noticeable gain in weight.
Personal financial Impact
The patient currently receives Social Security benefits that pay for treatment and medication however it is only $500 a month (not enough to live on). He has a hard time supporting himself and relies on his mother for help with housing, food, and transportation.
